Forms

There are various forms and information you need. Be sure to read the description and if it applies to you, download the form, fill it out, and turn it in to the Youth & Family Ministry offices.

Medical Release Form: If you are attending any youth event outside of normal class times, you need to have a Medical Release Form on file. They are good for one calendar year, so you only need to fill this out one time a year.

International Border Form: If you are going on a mission trip that is out of the US, you need to have this form; it gives you permission to cross international borders. 2008 forms will be available in January 2008.

International Medical Form: If you are going on a mission trip that is out of the US, you need this separate Medical Release Form that is trip specific. 2008 forms will be available in January 2008.
